Please read instructions:

Fill out entire application. Enter your name as you wish it to appear in your Certificate/Diploma. Please make sure that you enter an email that you will be able to have access during the Summer Program. We often send out communications via email.

In Section 9 you will be able to attach your resume and letter of interest or other documents, if applicable.

After you submit your application you will be taken to a different screen where you will be able to process your application fee payment of $85.00.

Note: Credit card payments will only work if the address submitted is the cardholder's address. If your address is not the same as the cardholder's address, please enter your address information on this form. You will then need to enter the cardholder's address on the payment site when you enter your credit card number.
