Event Planning

  • Alcohol Approval Form
    The Alcohol Approval Form must be completed before each event where alcohol will be served. Completed forms and appropriate documentation are submitted to the Dean's Office.   • Tax Exempt Certificates
    The Tax Exempt Certificate is used to purchase tangible property without paying sales and use tax.
